Claimant alleges a right shoulder injury, mental anguish, chronic fear, anxiety, depression, emotional turmoil, hyper vigilance, and excessive sensitivity to stimulus, particularly noise.This claimant was videotaped riding in a go-cart as well as riding on a small dirt bike.
